How registration works

Choose a doctor and fill in the application

Complete the web form below. One application per person. For a minor child, the parent or guardian completes the form.

Download the pre-filled PDF and sign digitally

After submitting the form you will receive a pre-filled application PDF to download.
Open it in DigiDoc4 (id.ee/tarkvara, free) or the RIA DigiDoc mobile app and sign with an ID card, Mobile-ID or Smart-ID.

Upload the signed application

After signing, upload the .asice file in the upload field on this page — it goes directly to our team.

We will accept you onto our patient list based on your application

Information about the family doctor change will appear in the health portal within the next calendar month. You will not receive a separate confirmation from us about being added to the list.

Newborns: A newborn is automatically added to the mother's family doctor's list (unless a different arrangement is requested). A separate application is only needed if you wish to register the child with a different doctor.

Transferring from another doctor: If you are joining us from another family doctor, please note your previous doctor's name in the comments field.
